How to verify a Illinois CNA license number
Illinois CNA license numbers can be verified either online, by email, over the phone, or by mail.
Online
To verify a CNA license online, visit the Illinois Department of Public Health’s Health Care Worker Registry page. Navigate to the Health Care Worker Registry section and search the registry by the individual’s name. Once you have selected a matching individual, you can verify that it is the right listing by entering the matching social security number.

How to lookup a Illinois CNA license number
The Illinois Nurse Aide Registry recently updated their search portal to include searchable healthcare workers, including nurse aides, in the state of Illinois. This has to be done by name and not by CNA license number, which can be completed on their website. If you don’t know the CNA license number and need it for any reason, here are a few things you can try:
- Call the Illinois Nurse Aide Registry at 844-789-3676 for instructions
- Request a replacement of your CNA certificate through the Illinois Nurse Aide Registry.
- Look for old correspondence from the Illinois Board of Nursing to see if your license number is there.
- Contact your CNA class provider to determine whether they have your CNA license number on file.
Why are CNA license numbers important?
A CNA license number is important because it is proof that you have completed the necessary training and testing to be a certified nurse aide in Illinois. Your CNA license number also allows your employer to verify your certification status with the Illinois Nurse Aide Registry.
How do I know if my CNA license is expired?
You can check the expiration date of your CNA license by logging in to the Illinois Nurse Aide Registry website. Once you are logged in, click on “My Profile” and then “View/Print Wallet Card.” Your CNA license expiration date will be listed at the bottom of the wallet card.
What to do if your Illinois CNA license has expired?
Illinois Certified Nurse Assistants who haven’t provided paid nursing or nursing-related services for at least 24 months will have their authorized certification revoked. Here’s how they may recertify:
- Pass a manual skills test given by an authorized Illinois evaluator.
- Take a written test to demonstrate knowledge of your CNA skills.
